PARENT AND STUDENT INTERNET CONSENT FORM
La Salle Academy has chosen to permit students access to the internet to further its educational goals and objectives. Reasonable care will be taken to assure the appropriateness of accessed material. However, parents are advised that La Salle Academy and the Diocese of Allentown do not have total control of the information on the internet. Parents are the primary authority responsible for imparting the standards of ethical and legal conduct their child should follow. Therefore, La Salle Academy supports and respects each family’s right to decide whether or not their child may have access to this resource.

I understand that students may not access the internet unless a teacher or teacher assistant is present. I also hereby indemnify and hold harmless the Diocese of Allentown and La Salle Academy from any claim or loss resulting from any infraction by the student of the policy or any applicable law.

LA SALLE ACADEMY COMPUTER POLICY
RESPONSIBILITIES
2008-2009
La Salle Academy has made a commitment to safeguard our children and the proper use of all technology equipment, including scanners, cameras, audio visual equipment, white boards, and handheld remotes.
La Salle Academy students will adhere to the following:
1. Any vandalism or attempted vandalism will result in immediate suspension from school. SEE ZERO TOLERANCE POLICY (August 1999)
2. Only La Salle Academy issued computer disks are permitted in school, unless the student has made other arrangements with Mrs. McNamara
3. Certain flash drives are not compatible with La Salle’s network.
4. Students may not access the internet unless a teacher or teacher assistant is present, and a signed consent form is on file.
5. La Salle students are not permitted to search unauthorized websites.
6. La Salle Academy’s network in unified and students are not permitted to change configuration settings, etc.
Violation of any of the above will result in parents being notified and the loss of technology privileges.
